4. Form CSD 1547 On the Individual Forms page, let's say that you want to search for Form CSD 1547. It is titled Notice of Change of Address. Rather than scroll through the entire Individual Forms page searching for the form number or form title, you can search for the CSD form number, the form name, or the issue date. The easiest method of searching is by form number. To do this in your browser window, simultaneously press the <CTRL> key + the letter F key. Then type the form number. (You do not need to include the letters CSD for form numbers.) Your cursor will jump to the next listing of that form number. Then click on the form number hyperlink in the PDF (or WordPerfect) column to open that verison of the form. Instructions Assume that we've searched for Form CSD 1547 and have found it listed in the N entries. Now we want to open its fillable PDF version.
