4. Form CSD 1547 (continued) Form CSD 1547 opens in Adobe Acrobat. Shown at the right is a representation of that form. Many of the court forms used in CM/ECF, such as Form CSD 1547, are fillable PDF forms that you can complete on your computer. To make entries on a fillable PDF form, simply click in an empty form field (generally indicated by a line or a check box), then begin typing your information. On the signature line of many of our court forms, you can electronically sign the document using the /s/ signature notation, followed by the signor's name. Example If John Doe was to electronically sign the form, his electronic signature would be typed as /s/ John Doe This notation indicates that the form was electronically signed by John Doe. Instructions Notice the areas highlighted in red at the right. They describe the fillable areas on the form and the use of the electronic signature notation of /s/.
