Filing an Amended Schedule

To amend a schedule, you are required to submit one original and one copy of the items shown in this table and a filing fee of $30 (exact amount only; cash, cashier's check, or money order).

  1. Amendment form (Form CSD 1100) with service completed as indicated on reverse side
  2. Notice to Creditors of the Debtor Added by Amendment or Balance of Schedules (Form CSD 1101)
  3. Amended petition and/or schedule(s)
  4. Creditor Matrix

If you are not accepting the conformed copy (exact duplicate of the filed document) in person, include a self-addressed, stamped envelope or messenger slip.

Filing a Motion for Relief from Stay

To file a Motion for Relief from Stay, you are required to submit one original and one copy of the items shown in this table and a filing fee of $150 (exact amount only; cash, cashier's check, or money order, or attorney's trust account check).

  1. Motion for Relief From Stay or
  2. Motion for Relief from Stay - Real and Personal Property (Form CSD 1160) or
  3. Motion for Relief from Automatic Stay - Unlawful Detainer (Form CSD 1163), plus
  4. Notice of Filing of a Motion for Relief from Automatic Stay (Form CSD 1185)

Filing a New Adversary Complaint

To file a Adversary Complaint, you are required to submit one original and one copy of the items shown in this table and a filing fee of $250 (exact amount only; cash, cashier's check, or money order, or attorney's trust account check).

  1. Original complaint, in pleading format, properly assembled and separate from the other required documents
  2. Adversary Proceeding Cover Sheet (Form B104)
  3. Summons in an Adversary Proceeding (Form CSD 3007)

  • For debtors or creditors: Complete the Notice of Change of Address (Form CSD 1547) and include one original and one copy
  • For attorneys: Complete the Attorney Change of Information (Form CSD 1546) and include one original and one copy

If you are being represented by an attorney, ask your attorney about filing electronically.

  • Remember to sign your documents.
  • Please staple documents in the upper-left corner only.
  • The court accepts payment of filing fees in cash, cashier's check, or money order and for the exact amount only. Personal checks are not accepted.

  • If you mail your documents to us, please include a self-addressed envelope with sufficient postage so that we can return a copy to you.
