Form Categories

Forms for Filing a Bankruptcy Case - A combination of different forms must be used to file a voluntary bankruptcy case, including a Bankruptcy Petition, Schedules, and Statements. Bankruptcy Petition Packages and all forms required for filing a bankruptcy case include basic instructions and all of the forms required to file a bankruptcy case.

Local Forms - Local forms are approved by the United States Bankruptcy Court - Southern District of California. Many are referenced in the Local Bankruptcy Rules and/or Administrative Procedures.

Official Bankruptcy Forms - Official Bankruptcy Forms are approved by the Judicial Conference of the United States to file and take action in bankruptcy cases and adversary proceedings. For more information, such as instructions and committee notes, please visit the Bankruptcy Forms page on the United States Courts Web site.

Director’s Forms - Director’s Bankruptcy Forms are issued under Bankruptcy Rule 9009 by the Director of the Administrative Office of the United States Courts. The use of Director’s Forms may be required by local court rules or general orders, but otherwise exist for the convenience of the parties.
